Downloadable British Passport Forms
The below provides links to the most common Passport forms required to apply for a British passport. Whilst all of these forms are also available via the Gov.uk official resource we have complied the most commonly used forms in one place for your convenience.
If you are simply renewing your passport with no name change or any other issues then it is likely that the only form that you will need is the Official Passport Application & Renewal form. There is no downloadable form for this. It must be the paper version which we can arrange to be delivered to you at no charge or you can collect it from the Post Office.
Free LS01 Lost & Stolen Form (PDF) – This is the form that you need to complete if your passport has been lost or stolen. This form needs to be sent to Her Majesty’s Passport Office if your passport was lost, stolen within the UK or to your local British Embassy if the passport was lost abroad. You should also report the loss or theft of your passport to your local Police station or the local authorities if you overseas. This form can be faxed or posted. You can complete this form on behalf of another providing you have their permission. Any fields that you do not know the answer to should be left blank.
Free Authorization Letter (PDF) – If you are attending someone elses appointment and they are over 16yrs and you are not their parent or guardian then the applicant will need to sign an Authorization Letter giving you permission to attend the appointment on their behalf. There are two versions of this letter. One is a print out “fill in the blanks” version which the Passport Office has created and can be found here. Another is an online version where by one of our agents will create the letter on your behalf and you will receive a copy via email, fax or in the post found here. There is no charge for this service.
Counter Signatories Guide (PDF) – It is important to state that only certain people are eligible to counter sign Section 10 of the application and the reverse of one of the photographs. Changes over years now mean that doctors and solicitors can no longer counter sign for applicants unless the applicant knows them on a social basis. The general rule of thumb is that a counter signatory must know the applicant for 2yrs and not be related by blood or marriage and be a British or Irish passport holder themselves. Passport Application within UK
Official Passport Application & Renewal Form – There is no downloadable Passport Application form when applying from within the UK you will need the 04 paper form available online or from the Post Office.

Credit/Debit Card Payment form for UK Application (PDF) – If you are are attending an appointment at the Passport Office then you can use your card on the day via the machine that they have avlaible on site. If you are applying through the post or wish to use someone elses card on the day of the appointment then you need to provide the Passport Office with a signed copy of this payment authorization form. Whilst this form does say the Foreign & Commonwealth Office it can be used to pay Her Majesty’s Passport Office.

Passport Application & Renewal Help Guide (Overseas) (PDF) – This official help guide provided by Her Majesty’s Passport Office will provide you with all of the information and advice required to successfully apply or renew your application from outside of the United Kingdom. The guide also provides information on how is and is not eligible for a British passport.

Credit/Debit Card form for Overseas Application (PDF) – If you are applying for a British passport from overseas and you are paying via credit or debit card then you will need to print out this payment form, complete your card details and send it to Her Majesty’s Passport Office along with your supporting documents and old passport (if this is a renewal).
Extend Current Passport by 12 Months (PDF) – If you are living overseas you can apply for your current British Passport to have its validity extended by 12 months. This service was brought in in July 2014 to combat the back log of applications that Her Majesty’s Passport Office incurred, which was reported to be the highest for 10yrs. This service can only be used if meet the eligible critea. See the form for details.
Emergency Travel Document Application (PDF) – If you passport has been lost, stolen or damaged overseas or you have found that it has simply expired and you need to travel urgently then you will need to apply for an Emergency Travel Document by completing this form and visiting the British Embassy in the country that you are in (appointment required). This temporary passport (once issued) will allow you to travel and return to the UK where it will be forfeited upon arrival into the United Kingdom. The fee for an ETD is around £95.00 payable in the currency of the country that you are currently residing in.
